I do not expect AI to replace expert review as a whole. Judging clinical relevance, understanding the realities of care in LMIC, recognising when technically sound recommendations are not feasible or equitable and weighing the significance of findings for children and families still require experience and contextual judgement. I see AI becoming a co-reviewer rather than an independent substitute for an expert.

 One point that I think deserves attention is the diversity of expert opinions. AI is a valuable scientific tool, but if all reviewers rely too heavily on the same AI-generated reasoning, we may gradually lose the diversity of perspectives that makes peer review so valuable. Every expert brings different experience, disciplinary knowledge, scientific culture, and critical thinking. This individual contribution is the real added value of expert review and should always be preserved. AI should strengthen this diversity, not reduce it.
